Brugada syndrome is a rare genetic disorder mostly affecting young subjects without any underlying heart disease. Here we are describing 1 patient presented for gastric bypass surgery who had near cardiac arrest under general anesthesia for unknown reason. Postoperative investigation of this case revealed the diagnosis of Brugada syndrome. •Brugada syndrome.•Anesthetic management.•Near cardiac arrest.•High-coved ST segment.
