Abstract
The giant omphalocele is currently a surgical challenge. The morbidity and mortality associated with its care is non-negligible. Nowadays, different studies have revived the debate between conservative and surgical management for giant omphalocele. The purpose of this study is to compare the conservative and surgical management of the giant omphalocele in terms of morbidity and mortality.
Retrospective study including all giant omphaloceles comparing surgical management (French University hospital centers) and tanning (Ivory Coast University hospital center). Epidemiology was studied as well as medical and surgical managements both intra and post operative.
One hundred and forty-seven patients included (98 patients in the “tanning” group and 49 in the “surgery” group). Hospital length of stay is significantly shorter in the “tanning” group as they do not spend time in intensive care unit. Morbidity is higher in “surgery” group. The average duration for oral empowerment was acquired at 179 days in the “surgery” group, whereas in the “tanning” group 90% was immediately and exclusively breastfed. No significant differences in terms of epithelialization time.
The tanning treatment has its own place in the therapeutic arsenal in the management of the giant omphalocele no matter where it takes place. However, its realization in surgical environments prevents certain complications related to the technique or the pathology.
L’omphalocèle géante est actuellement un défi chirurgical. La morbidité et la mortalité associé à sa prise en charge sont non-négligeables. Actuellement, différentes études ont relancé le débat entre la prise en charge conservatrice et chirurgicale de l’omphalocèle géant. Le but de cette étude est de comparer le traitement conservateur et chirurgical dans l’omphalocèle géante en termes de morbidité et la mortalité.
Étude rétrospective incluant toutes les omphalocèles géant gérées par fermeture chirurgicale (centres hospitaliers universitaires français) ou par tannage (centre hospitalier universitaire ivoirien). L’épidémiologie a été étudiée aussi bien que la gestion médicale et chirurgicale per- et postopératoire.
Cent quarante-sept patients ont inclus (98 patients dans le groupe « tannage » et 49 dans le groupe « chirurgie »). La durée d’hospitalisation est significativement plus courte dans le groupe « tannage » avec une absence de passage aux soins intensifs. La morbidité est plus haute dans le groupe « chirurgie ». La durée moyenne pour l’autonomisation orale est de 179jours dans le groupe « chirurgie », tandis que dans le groupe « tannage » 90 % des enfants ont été immédiatement et exclusivement allaités. Aucune différence significative en termes de temps d’épithélialisation n’est à noter.
Le traitement par tannage a sa place dans l’arsenal thérapeutique de l’omphalocèle géantes. Cependant, sa réalisation dans un environnement chirurgical permet la prise en charge des complications liées à la technique ou à la pathologie.
