Abstract
The aim of the present study was to assess the experiences of Pharmacists related to the type and extent of prescription errors to evaluate the attitude of health professionals towards pharmacist's intervention in patient care activities. A quantitative cross-sectional study based on review of pharmacist's experiences about medication errors was conducted. Questionnaires were distributed among 100 pharmacists working in 8 different private hospital settings from June 2013-December 2013. The study revealed that 87% of pharmacists had interaction with health professionals, most of the time they enquired about availability as well as the alternative of medicines. Poor legibility, incomplete inscription, wrong combination of antibiotics, lack of dose adjustment, etc. were observed by pharmacists during wards round and in dispensing prescriptions. Frequent occurrence of medication errors has necessitated the induction and collaboration of pharmacists with other health professionals in hospital settings. They must develop a fruitful working environment to curb the life threatening events.
