Abstract
La généralisation de la prophylaxie post-natale par les immunoglobulines anti-D a permis de diminuer l’incidence de l’incompatibilité fœto-maternelle érythrocytaire (IFME) dans le système Rhésus. Peu d’études récentes se sont intéressées à évaluer la situation de la prophylaxie anti-D en Tunisie et à étudier ses répercussions sur la santé maternelle et néonatale. Nous avons conduit une étude rétrospective descriptive des cas d’IFME dans le système rhésus dans le service de médecine et de réanimation néonatale du Centre de maternité et de néonatologie de Tunis durant la période du 1er janvier 2006 au 31 décembre 2015. Nous avons colligé 51 cas d’IFME. La prévalence de l’IFME a été de 3,4 pour 10 000 naissances vivantes et 41 % des patients avaient été transférés in utero, leurs origines étant en premier lieu le Nord-Ouest du pays (17 %). Le taux de femmes ayant un antécédent d’au moins une fausse couche spontanée était de 45 %. Dans 42 % des cas, les grossesses avaient été suivies dans des dispensaires locaux. La recherche d’agglutinines irrégulières avait été réalisée chez 86 % des femmes. Elle avait été positive dans 97 % des cas. La prophylaxie anti-D avait été correctement conduite dans seulement 27 % des cas. La prophylaxie systématique du 3e trimestre, ainsi que la réalisation systématique du test de Kleihauer doivent être couplés à la prophylaxie du postpartum pour mieux identifier les dysfonctionnements et proposer une amélioration de l’application des recommandations.
Generalization of postnatal prophylaxis using anti-D immunoglobulins decreased the incidence of erythrocyte fetal–maternal incompatibility (EFMI) in the Rhesus system. Few recent studies have investigated the situation of anti-D prophylaxis in Tunisia and its effects on maternal and neonatal health. The aim of this study was therefore to analyze the situation of anti-D prophylaxis in Tunisia to detect defects and propose solutions. We conducted a retrospective descriptive study of IFME cases in the rhesus system in the Department of Medicine and Neonatal Resuscitation of the Tunis Maternity and Neonatology Center (CMNT) during an 8-year period from 1 January 2006 to 31 December 2015. We collected 51 cases of IFME. The prevalence of IFME was 3.4 per 10 000 live births; 41 % of the patients were transferred in utero and they were from the northwest of the country (17 %). The rate of women with a history of at least one spontaneous miscarriage (SCF) was 45 %. In 42 % of the cases, pregnancies were monitored at local clinics. The search for irregular agglutinins (RAI) was performed in 86 % of the women studied. RAI was positive in 97 % of the cases. Anti-D prophylaxis was correctly performed in only 27 % of the cases. The systematic prophylaxis of the third trimester and a systematic Kleihauer test must be combined with postpartum prophylaxis to better identify dysfunctions and improve the application of the recommendations.
