Abstract
La sarcoïdose est une maladie granulomateuse d’étiologie indéterminée, caractérisée par la présence de granulomes sans nécrose caséeuse et touchant plusieurs organes. L’atteinte gastro-intestinale est rare. L’atteinte gastrique et en particulier antrale représente la localisation extrahépatique la plus fréquente. Nous rapportons quatre cas de sarcoïdose gastro-intestinale.
Étude rétrospective d’une série de quatre observations.
La sarcoïdose était de siège gastrique chez les quatre patients. Cette localisation était associée à une atteinte duodénale, iléale et de la vésicule biliaire chez une patiente aux antécédents de pancréatite aiguë, probablement d’origine sarcoïdosique. La maladie était révélée par des douleurs abdominales modérées dans deux cas, par un syndrome obstructif associé à un amaigrissement important et à une entéropathie exsudative chez la patiente ayant une atteinte digestive multiple et par une hémorragie digestive dans le dernier cas. L’estomac était endoscopiquement normal dans un cas, présentait une congestion antrale dans un cas, une gastropathie ulcérée chez le patient ayant présenté une hémorragie digestive et avait un aspect de pseudo-linite chez la patiente ayant un syndrome obstructif. L’iléon et le duodénum étaient d’aspect endoscopiquement normal chez la patiente ayant une atteinte digestive multiple. Le traitement a consisté en une antrectomie suivie d’une corticothérapie chez la patiente ayant un syndrome obstructif. L’exploration peropératoire a permis de découvrir un ulcère duodénal perforé et bouché par la vésicule biliaire. Il a consisté en une association corticothérapie et inhibiteurs de la pompe à protons chez le patient ayant présenté une hémorragie digestive. Les deux autres malades n’ont pas été traités devant la disparition spontanée des douleurs. L’évolution était favorable dans tous les cas.
L’estomac est l’organe le plus fréquemment touché dans la sarcoïdose gastro-intestinale. La sarcoïdose gastrique peut mimer une néoplasie se révéler par une hémorragie digestive haute. L’atteinte du duodénum, de l’intestin grêle et du côlon est rare, mais son incidence est sous-estimée car les biopsies ne sont pas systématiques.
Sarcoidosis is a granulomatous disorder of unknown cause, characterised by noncaseating granulomas affecting multiple organs. Gastrointestinal tract involvement in sarcoidosis is rare. The stomach, particularly the antrum is the most common extra-hepatic organ to be involved. We report four cases of gastro-intestinal sarcoidosis.
Retrospective study of a series of four cases.
All patients had gastric sarcoidosis. It involved the duodenum, ileum and gall bladder in a patient with a history of an acute pancreatitis probably due to sarcoidosis. This patient presented with obstructive intestinal manifestations, weight loss and exsudative enteropathy. Two patients presented with mild abdominal pain and the last patient was admitted for upper gastrointestinal bleeding. The endoscopy was normal in one case and showed an antral congestion in another case. Gastric ulcers were found in the patient with a history of upper gastro-intestinal bleeding. A pseudo-linitic aspect was noticed in the patient with obstructive manifestations. The duodenum and the ileum were normal. This patient had an antrectomy and was treated with corticosteroids. Surgery evidenced a perforated duodenal ulcer, which was obstructed by the gall bladder. The patient with gastrointestinal bleeding received proton pump inhibitor and corticosteroids. These two patients improved gradually. The two other patients recovered spontaneously.
The stomach is the most commonly affected organ in gastrointestinal sarcoidosis. Gastric sarcoidosis can mimic a malignant lesion owing to narrowing of the gastric lumen or can be revealed by upper gastrointestinal bleeding. Duodenum, small bowel and colon involvement is uncommon but may be underestimated in the absence of systematic biopsies.
