• Mesenteric, splenic and portal veins thrombosis is a rare presentation after laparoscopic sleeve gastrectomy. • Abdominal pain after laparoscopic sleeve gastrectomy can be due to mesenteric, splenic and portal veins thrombosis. • Mesenteric, splenic and portal veins thrombosis can be managed properly with low-molecular-Wight heparin (LMWH, enoxaparin).
